Q: The overall length in the description says 12', and the blade length 3.1', but this knife looks to be more or less half blade/half handle. How long is

12 feet? 3 foot blade? sounds more like a sword

The blade and handle are from the same piece of metal - the knife is one solid piece.

Q: Does this fit well on the back of the belt? Does the sheath support wearing it this way?

You can wear it horizontally on a narrow belt, but this is probably one of the worst designed sheaths that I've ever encountered. As much as I'd like to, I haven't even been able to carry this, otherwise, superb knife. They should've incorporated a regular boot clip to the sheath for more well-rounded carry options.
