Q: Will this hold a regular notebook?

I am not entirely sure what you mean by "notebook." It holds my iPad Air with no problem. It would work with a wide variety of electronic devices but if you mean an actual paper notebook probably not.

Q: Will this securely hold a phone, galaxy s21, horizontally, with clear access to the usb port?

Yes, you can use it horizontally. I use it with my s10+, and is adjustable to fit the length of the s21. The bottom holding bracket will spread outward and leave plenty of room to attach a charging cable.

Q: Is there a case or carrying bag ?

Dear customer, the carry bag is not included in the package.

Q: why are the videos shows of a different model. does this unit i see come with extending arm or not?

It did not come with an extension arm. But it is secure and sturdy. I have it mounted on my drum kit at church to affix my iPhone which controls the virtual soundboard and also provides tempo for the drums.
