Is there an instruction book for this grid?

No, You should buy the instruction book or CD if you need. There is a small piece of printed picture. I leaned how to use from my personal trainer. If you are a stranger of grid, you should find how you can use it properly first.

Where is the country of origin?

the roller itself was manufactured in Taiwan but the corporate office is in Texas. I believe the concept and development of this product was created by a personal trainer from the USA.

I purchased the Trigger Point Performance Grid 2.0 EVA Foam Roller. It was working great, but split into 2 pieces. Is it defective ?

Mine broke in half also. Bad glue jobs I believe. I actually like it better in the smaller configuration as I can use 2 halves to do the back (they are wide enough) and they become easier to transport. If you have some reason that you like it that big, ask for a replacement. (I can do my back and both legs at the same time with the half width.)

I have both the Grid 2.0 and the original. I use the 2.0 at home, since its larger and I have more space. The original is great for traveling and tossing in the gym bag.
