does it include a PoE adapter?

Mine did not. I was upgrading another Hikvision I already had in place, so used it's adapter. I have 4 of this brand now and have been quite happy to this point. I had to purchase the adapters for all of them separately as none of them came with it. I'll probably move up to a POE switch next instead of dealing with so many scattered injectors.

will this cam work with a Lorex nvr 4k ?

Only if the Lorex system has an ONVIF feature to be enabled. Other than that, LOREX keeps there encoding proprietary.
