palm is smaller that 3- 4 inches. can I still use this?

If by palm you mean the area between the base of your fingers, you should be able to. It depends on just how small that area is. You can always return it if it doesn't fit.

Would this work to prevent hyper extension from pressure

Its very awkward to apply to the hand and within minutes it falls off. If you adjust the straps to tight the circulation gets cut off. It has to be monitored constantly. The patient can wear it about 10 minutes then it falls off

I didn't notice until now, but the cover zips off for washing. My physical therapist bent it to fit my needs so it's flexible, too. I'm very pleased with it. I had trigger finger and used it to keep my hand flat when I slept. It's also very comfortable.
