No, It does not require a power supply. This POE extender is powered by PoE devices (48V or 57V PoE Switch/PoE NVR/PoE Injector).

How many PoE extenders are needed if I want extend transmission distance up to 800ft.

3 PoE extanders are required. Each extender supports an extension of 328 ft and 3 cascades, so the 3 poe extenders can extend a total of 984 feet distance.
