Can this be used on a potted blueberry bush?

EM-1 can be used on ANY plant. It won’t hurt your blueberries. It activates the soil with beneficial microbes, which in turn make the nutrients in your soil available to the plant by breaking them down into substances the plant can access and use. It’s perfectly safe.

How long does it last after mixing with water?

If you are mixing the EM1 to water for use only - you want to use it that same day. If you are talking about activating the product for use - that is a whole different, detailed answer.
