If I put the device against my arm, do I have to put my finger in my friends ear for them to hear it or can I touch a different part of their head?

I've never tried that, but generally this device works by transmitting the acoustic wave through hard surfaces like bone. Human flesh doesn't transmit the sound well. The little bit of skin between the transducer and your skull is one thing, but the situation you describe doesn't seem like it would work.

Can you glue this to underside of massage table?

Yes. However, I would recommend placing it against a hollow surface for the loudest effect.

Could you, in theory, cut the speakers out of an old pair of earbuds, and solder these on?

It’s heavy, so it is not a good idea to do so. In addition, two terminals are fragile, got broken easily.

Is it possible to hear this through your bones by tightly gripping a handle its imbedded in?

It's possible to hear if you can get max volume through the device. It's better quality on head or skull. I was able to hear by placing on my ribs as well as arm. If your going to use as a headphone hooked up to a phone or mp3 then probably not. The only way I was able to hear audio from other parts of my body was with a headphone amplifier. The tighter the device is to the bone the better the sound.
