How easy is it to clean?

I run 2-3 of them all day every day for 6 months of the year. Any humidifier is going to get build up on it, especially the red/pink bacteria. Tap water will cause sediment build up but if you use purified water or distilled water it will hardly have any sediment build up. Tap water will also put sediment in the air that plugs air filters like your HVAC air filter (depending on merv). I have a reverse osmosis filter that I use to fill my humidifiers up with, (cheapest method to fill humidifiers I've found) plus you get filtered drinking water on the side, no running to the store all the time and hauling water back and forth. And since reverse osmosis cleans the water of crusty sediment build up (I never actually get any) then all I have to do is take care of the bacteria or red/pink growth. I drop a (few drops) of colloidal silver into each refill (30ppm) and I never see the bacteria anymore.

the answers are inconsistent - does this humidifier use a filter that you have to either clean or replace?

There IS NOW a filter the company is promoting. If your humidifier is new a little piece of paper comes with it saying if you follow a link on their website you can get one for free. For those of you who already own, you could call the manufacturer and ask about it.

Do you need to use distilled water?

100% YES. You should never use an ultrasonic humidifier with anything except distilled water. Otherwise, you will be spraying mineral molecules into the air, and your pm2.5 readings (a measure of air pollution) will skyrocket. This is extremely dangerous to your health in the long term, like living in a heavily polluted city.

We just bought this unit and we have accidentally tipped it over once and had a couple close calls. Anyone else have this issue?

The same thing happened to us but it happened coz the unit started leaking. I called Customer Service & their small team is so helpful. They sent us a replacement because the cracking due to fall isnt covered in their warranty, but the leaking is & it was a defective unit as per them. 100 stars to their customer service!!
