Just curious, did anyone here have a real lightning strike with this device installed? What happened?

I had lightning strike the ground/grass within 5 feet of my wireless equipment. The wireless fired because of the energy in the air. The fuse took the rest of the hit in my closest. The fuse stropped the damage from getting to the rest of the equipment. I think it did its job of protecting the rest of the equipment. It is a one time use type of thing when it gets damaged like this. I replaced it with the same model and moved on. I am happy.

Does this cause a slowdown in network speed?

I have a gigabit router and did not notice any slowdown in speed.

How do you hook this up? Do you need two cables? Ground it to an outlet box - take off the plastic cover on the outlet box & attached cable to screw?

there is a ground wire on the device that needs to be connected to a ground. You could use an outlet, conduit or other ground source. The closer your surge protector is to the ground rod, the more efficient it will be at protecting your device.

Weatherproof/outdoor rated?

Likely not outdoor rated, but the container would be weather "resistant" however as Allan comments, the connectors are not weatherproof or waterproof or even "resistant." If it is used outside, it will need to be put into a weatherproof container.
